Think about the human brain as a magnificent, finely tuned orchestra. Every neurotransmitter, every receptor, and every synapse plays a precise note to keep us in a state of balance—what we call homeostasis. But when a person introduces massive amounts of external chemicals into this system, the conductor gets thrown off the podium. The brain is forced to adapt to this new, artificial normal. When that substance is suddenly removed, the orchestra descends into absolute chaos.

As a practical nurse, you are stepping into that chaos. You are there to keep the patient safe and, through the sheer physics of human connection, guide them back to reality.
When a client with a substance use disorder first arrives on your unit, we have two immediate jobs. First, we deal with the reality of addiction: Nurses should assess for hidden illicit substances or drug paraphernalia upon admission of a client with a substance use disorder. People hide things out of fear and compulsion; our job is to ensure a safe environment, not to pass judgment. Second, we must look at the physical toll. The pursuit of the substance often replaces the pursuit of food. Therefore, nutritional status must be assessed and managed in clients with substance use disorders due to common dietary neglect.
Once the baseline is established, we have to manage the chemical storm. Let’s break down exactly how these substances hijack the body, and how we take the control back.
Nature abhors a vacuum, and the brain abhors a sudden drop in depressants. Alcohol and benzodiazepines suppress the central nervous system (CNS). If you chronically suppress a spring and suddenly let go, it violently snaps back. That "snap back" is withdrawal.
Alcohol: The Coiled Spring
When a dependent client stops drinking, the brain wakes up in a hyperactive panic. Timing is everything here: Alcohol withdrawal symptoms typically begin within 4 to 12 hours after the last drink.
Because the central nervous system is rebounding into overdrive, the body’s engine revs dangerously high. Signs of alcohol withdrawal include tremors, tachycardia, diaphoresis, and elevated blood pressure. We do not just guess how bad this is; we measure it. The Clinical Institute Withdrawal Assessment for Alcohol (CIWA) scale dictates the dosage of benzodiazepines administered to a client. To calm that rebounding CNS, Chlordiazepoxide is a long-acting benzodiazepine commonly used to prevent seizures during alcohol withdrawal.
If we fail to control this rebound, the client enters the extreme, life-threatening end of the spectrum. Delirium tremens is a severe form of alcohol withdrawal characterized by hallucinations, severe agitation, and autonomic instability.

Feynman Rule of Thumb: When the brain is agitated, the environment shouldn't be. A quiet, low-stimulus environment reduces agitation in clients experiencing substance withdrawal. Keep the lights dim and the noise down.
We also have to look at the long-term brain damage alcohol leaves behind. Alcohol decimates the body's absorption of B vitamins. Wernicke-Korsakoff syndrome is a neurological disorder caused by a severe thiamine deficiency. It literally damages the brain's memory centers. Because of this, thiamine replacement is administered to clients with chronic alcohol use to prevent Wernicke-Korsakoff syndrome.
Medical Management for Alcohol Abstinence
How do we help them stay sober? We have two fascinating pharmacological tools:
- The Punisher: Disulfiram is a medication used to deter alcohol consumption by causing severe nausea and vomiting upon alcohol ingestion. It is incredibly strict! Clients taking disulfiram must avoid all products containing alcohol including mouthwash and vanilla extract.
- The Craving Killer: Naltrexone is an opioid antagonist used to decrease cravings for both alcohol and opioids. It blocks the euphoric reward pathways in the brain.
Benzodiazepines: The Chemical Cousins
Benzodiazepines act on the exact same inhibitory pathways as alcohol. If someone takes too much, they slump into a dangerous stupor. Benzodiazepine intoxication causes slurred speech, ataxia, and central nervous system depression.
If you find a client unresponsive from a benzo overdose, remember your ABCs: Nurses must assess the airway and respiratory status frequently during suspected central nervous system depressant overdoses. If we need to reverse it, we use the specific antidote. Flumazenil is the specific pharmacological antidote for a benzodiazepine overdose.

What happens if they abruptly stop taking benzos? The same violent "snap back" we see in alcohol. Abrupt discontinuation of chronic benzodiazepine use can precipitate life-threatening withdrawal seizures. You cannot just stop them cold. Medical management of benzodiazepine withdrawal requires a gradual tapering of the medication to prevent seizures.
Crucial Precautions for Depressants
Because of the risk of neurological instability, you must implement two absolute safety measures:
- Seizure precautions must be implemented for clients undergoing severe alcohol or sedative withdrawal.
- Fall precautions must be implemented for clients experiencing intoxication or withdrawal due to impaired coordination and altered mental status.
Opioids don't just kill pain; they kill the drive to breathe.
Opioid Intoxication
If a client comes in heavily intoxicated on heroin, fentanyl, or prescription painkillers, look for the classic triad. Signs of opioid intoxication include pinpoint pupils, respiratory depression, and a decreased level of consciousness.

If they stop breathing, you must administer the reversal agent immediately: Naloxone is the specific pharmacological antidote for an opioid overdose. It knocks the opioid right off the brain's receptors.

Opioid Withdrawal: The Worst Flu Imaginable
When naloxone is given—or when the client just runs out of their drug—withdrawal begins. Opioid withdrawal looks entirely different from alcohol withdrawal.
Opioid withdrawal symptoms include rhinorrhea, piloerection (which causes the skin to resemble a plucked bird—literally where the phrase "cold turkey" comes from), muscle aches, diarrhea, and yawning.

Here is the secret to the NCLEX and real-world nursing: While alcohol withdrawal can kill you, opioid withdrawal is intensely uncomfortable and generally not life-threatening. It is agonizing, but it is rarely fatal.
We quantify this misery so we can treat it. The Clinical Opiate Withdrawal Scale (COWS) is used to determine the severity of opioid withdrawal and guide pharmacological treatment.
Long-term Opioid Management
We don't want clients stuck in this agonizing cycle, so we use replacement therapies to stabilize their brain chemistry:
- Methadone is a long-acting synthetic opioid used for opioid maintenance therapy and withdrawal management.
- Buprenorphine is a partial opioid agonist used to treat opioid use disorder by reducing cravings and withdrawal symptoms.

Cocaine, amphetamines, and methamphetamines dump massive amounts of dopamine and norepinephrine into the brain. The sympathetic nervous system is trapped in a state of "fight or flight."
Stimulant Intoxication
Stimulant intoxication presents with tachycardia, dilated pupils, elevated blood pressure, and hypervigilance.
The human heart is not designed to run a marathon while the body is sitting still. Severe stimulant intoxication can precipitate fatal cardiac arrhythmias, seizures, and hyperthermia. Because the heart is under such immense electrical strain, continuous cardiac monitoring is indicated for clients experiencing severe stimulant toxicity due to the risk of arrhythmias.

Stimulant Withdrawal: The Crash
What goes up must come down. When the stimulant is depleted, the brain is left completely devoid of dopamine. The client crashes hard. Stimulant withdrawal symptoms include profound fatigue, increased appetite, and severe depression.
Because this dopamine depletion leaves the client feeling utterly hopeless, your priority shifts dramatically. The primary nursing intervention for a client experiencing stimulant withdrawal is observing the client for suicidal ideation.
All the pharmacology in the world won't matter if the client doesn't trust you. Addiction is a disease of isolation; communication is the bridge back.
The Foundation of Trust
First, check your own biases at the door. Nurses must maintain a non-judgmental attitude when caring for clients with substance use disorders to foster trust. However, being non-judgmental doesn't mean letting the client run the unit. These clients often test limits. Therefore, setting clear boundaries is essential to maintain a professional and therapeutic nurse-client relationship.
The Tools of Connection (Therapeutic Techniques)
How do we actually talk to them? We don't interrogate; we investigate.
- Therapeutic communication relies on open-ended questions to encourage the client to share thoughts and feelings. Give them the steering wheel of the conversation.
- Active listening involves maintaining eye contact, leaning forward, and providing nonverbal cues of engagement. Your body language speaks before your mouth does.
- The technique of reflection directs client questions or feelings back to the client to encourage self-exploration. (Client: "I don't know how to stay sober." Nurse: "You're feeling unsure about your ability to maintain sobriety.")
- Restating involves repeating the main idea of a client statement to confirm nursing understanding.
- Validation acknowledges client feelings as legitimate and promotes a sense of being understood. You don't have to agree with their actions to validate their pain.
- Silence is a therapeutic communication technique allowing the client time to organize thoughts and process emotions. Do not rush to fill the quiet spaces. Sometimes, the most profound breakthroughs happen in silence.
- Empathy requires understanding the client perspective without absorbing the actual emotions of the client. If you jump into the emotional quicksand with them, you both sink. Throw them a rope instead.
- Confrontation in therapeutic communication involves gently pointing out discrepancies between client words and actions. (Nurse: "You mentioned wanting to get healthy, but I noticed you skipped group therapy today.")

The Traps to Avoid (Non-Therapeutic Blocks)
Just as there are correct ways to communicate, there are conversational landmines that will instantly destroy trust:
- Asking why questions often makes clients feel defensive and impairs the therapeutic nurse-client relationship. (Instead of "Why did you drink?", ask "What was happening right before you took a drink?")
- False reassurance minimizes client feelings and blocks further therapeutic communication. Never say, "Everything is going to be fine." You don't know that, and it tells the client you aren't taking their fear seriously.
- Changing the subject during a client expression of feelings is a non-therapeutic communication block. It tells the client that their emotional pain is too inconvenient for you to deal with.
Final Thoughts for the NCLEX-PN
When you look at a substance use disorder question on the exam, ask yourself: What is the chemical doing to the brain right now? Is the depressant being taken away? Guard against seizures. Is the stimulant wearing off? Guard against suicide. Is the opioid suppressing the brainstem? Protect the airway. And above all, no matter what they took, treat them with the precise, deliberate, and bounded empathy that defines exceptional nursing.
