The National Council Licensure Examination for Practical Nurses (NCLEX-PN) is a computerized adaptive test designed to measure the competencies required for safe and effective practice as an entry-level licensed practical or vocational nurse. Based on the framework of Client Needs, the examination assesses knowledge across diverse clinical domains, including coordinated care, infection control, pharmacological therapies, and physiological adaptation. Client
An individual, family, or group, which includes significant others and populations.
Order
An intervention, remedy, or treatment as directed by an authorized primary health care provider.
Prescription
An intervention specifically related to medication, as directed by an authorized primary health care provider.
Primary Health Care Provider
A licensed health care team member authorized to formulate prescriptions and orders (e.g., medical physician, advanced practice nurse).
Unlicensed Assistive Personnel (UAP)
Unlicensed personnel trained to function in a supportive role, to whom a specific nursing responsibility may be delegated.

Frequently asked questions
Good to know
- Every exam includes 18 specialized items grouped into three clinical judgment case studies. These assess the six steps of the NCSBN Clinical Judgment Measurement Model.
- Candidates cannot return to a previous question. Once an answer is confirmed and the 'NEXT' button is pressed, the decision is final.
- The exam includes partial-credit scoring models for multiple response questions (e.g., plus/minus scoring, zero/one scoring, or rationale scoring).
- Test length is not an indicator of pass or fail status. An exam may shut off at 85 items or go all the way to 150 items while evaluating the 95% Confidence Interval Rule.
